Overview

Serum 25-hydroxyvitamin D is the standard measure of vitamin D status, reflecting both sun-derived and dietary/supplemental vitamin D. Vitamin D acts as a prohormone important for calcium and bone.

Why it matters

Frank deficiency impairs bone health and causes rickets or osteomalacia, so identifying and correcting low levels matters. Beyond deficiency, however, high-quality trials show limited benefit from supplementation for most other outcomes — an important anti-hype point.

< 20 ng/mL (50 nmol/L) is commonly defined as deficiency and ≥ 20 as adequate for most people; some bodies use ≥ 30 ng/mL for 'sufficiency'.

Thresholds are debated between guideline bodies; routine screening of healthy people is not universally recommended.

Clinical interpretation

Low 25-OH-D indicates deficiency worth correcting, especially for bone health. Values well above the sufficiency range add no proven benefit, and very high levels from over-supplementation can cause harm (hypercalcemia).

Should everyone supplement vitamin D?
