Overview
Fasting insulin is the level of the hormone insulin after an overnight fast. It reflects how hard the pancreas is working to keep glucose in range.
Why it matters
Insulin resistance often develops years before glucose rises: the pancreas compensates by secreting more insulin, keeping glucose normal for a time. A high fasting insulin can therefore flag early metabolic dysfunction that glucose alone would miss.

Where the evidence points
There is no universally agreed 'optimal'; many labs report up to ~25 µIU/mL as normal, while lower values (in the absence of hypoglycemia) generally reflect better insulin sensitivity.
Insulin assays are not standardized across labs, so absolute values and cutoffs vary; trends and paired glucose are more informative than a single number.

Higher fasting insulin with normal glucose suggests compensated insulin resistance. It is a research and context tool rather than a formal diagnostic criterion, and is best read alongside glucose (see HOMA-IR).
